Overview

Pentoxifylline + vedolizumab is an investigational combination therapy studied for the treatment of Crohn's disease. Pentoxifylline is a synthetic methylxanthine derivative with hemorheological, anti-inflammatory, and anti-oxidative properties that interferes with tumor necrosis factor (TNF) gene expression. Vedolizumab is a humanized monoclonal antibody that specifically binds to the α4β7 integrin on lymphocytes, blocking their interaction with mucosal addressin cell adhesion molecule-1 (MAdCAM-1), thereby inhibiting gut-selective lymphocyte trafficking and reducing gastrointestinal inflammation. The rationale for combining these agents is to target multiple inflammatory pathways in Crohn’s disease—pentoxifylline modulating TNF-mediated inflammation and vedolizumab blocking T-cell migration into intestinal tissue. In a randomized pilot study, this combination was found to be safe but did not provide significant benefit over vedolizumab monotherapy in achieving clinical or endoscopic remission; however, it appeared to improve clinical response rates[1][2][3].
