Drug intelligence / Profile preview
Pirtobrutinib + methotrexate + immunochemotherapy is an investigational combination regimen being evaluated for the treatment of diffuse large B-cell lymphoma (DLBCL) with central nervous system (CNS) involvement. The regimen combines pirtobrutinib, a highly selective, noncovalent (reversible) Bruton's tyrosine kinase (BTK) inhibitor developed by Eli Lilly, with high-dose methotrexate (an antifolate chemotherapy) and standard chemoimmunotherapy (typically rituximab-based regimens). Pirtobrutinib targets BTK to block B-cell receptor signaling and downstream NF-κB activation, which is constitutively active in CNS DLBCL. Methotrexate provides potent cytotoxic activity with excellent blood-brain barrier penetration, and the chemoimmunotherapy component offers systemic anti-tumor efficacy. This combination is designed to leverage synergistic therapeutic activity to address aggressive and difficult-to-treat lymphoma in the CNS.
