Overview
Pola-ZR-Glo is an experimental, chemo-free combination regimen consisting of polatuzumab vedotin, zanubrutinib, lenalidomide, and glofitamab. This regimen is being evaluated by Ruijin Hospital for the treatment of older, treatment-naive patients with large B-cell lymphoma (LBCL) who are unfit for or decline standard immunochemotherapy. The combination leverages multiple distinct mechanisms: polatuzumab vedotin is a CD79b-directed antibody-drug conjugate (ADC); zanubrutinib is a potent and selective Bruton's tyrosine kinase (BTK) inhibitor; lenalidomide is an immunomodulatory agent targeting the cereblon E3 ubiquitin ligase complex; and glofitamab is a bispecific monoclonal antibody that recruits T cells to CD20-positive B cells. The regimen also incorporates a single pre-treatment dose of obinutuzumab to mitigate the risk of cytokine release syndrome (CRS) associated with glofitamab administration.
