Polatuzumab vedotin + bendamustine + rituximab (Pola-BR) is a combination chemoimmunotherapy regimen being evaluated for the treatment of relapsed or refractory mantle cell lymphoma (MCL), particularly in patients who have failed prior Bruton's tyrosine kinase (BTK) inhibitor therapy. The regimen consists of polatuzumab vedotin, a CD79b-directed antibody-drug conjugate; bendamustine, a bifunctional alkylating agent; and rituximab, a CD20-directed monoclonal antibody. This specific study is led by the Czech Lymphoma Study Group (CLSG) as a Phase II trial to address the high unmet need in post-BTKi MCL patients. Polatuzumab vedotin delivers the microtubule-disrupting agent monomethyl auristatin E (MMAE) directly to CD79b-expressing B-cells, while rituximab induces immune-mediated cell death and bendamustine causes DNA cross-linking.
