Polatuzumab vedotin + glofitamab is a chemotherapy-free combination regimen being investigated for the treatment of relapsed or refractory diffuse large B-cell lymphoma (DLBCL). Polatuzumab vedotin is a CD79b-directed antibody-drug conjugate (ADC) consisting of a humanized anti-CD79b monoclonal antibody conjugated to the microtubule-disrupting agent monomethyl auristatin E (MMAE) via a protease-cleavable linker. Glofitamab is a bispecific T-cell-engaging antibody with a unique 2:1 molecular structure, containing two binding sites for CD20 on B cells and one binding site for CD3 on T cells. The combination aims to provide synergistic antitumor activity by combining direct tumor cell killing (via MMAE-induced microtubule disruption) with T-cell-mediated cytotoxicity. Clinical trials, including those sponsored by Yonsei University and Roche, are evaluating this regimen as a salvage therapy for patients with aggressive B-cell lymphomas, including those who have failed prior R-CHOP or CAR T-cell therapy.
