Polatuzumab vedotin + miniCHP + obinutuzumab is a combination chemoimmunotherapy regimen primarily investigated as an induction backbone for the treatment of diffuse large B-cell lymphoma (DLBCL), particularly the MCD subtype. The regimen consists of polatuzumab vedotin, an antibody-drug conjugate (ADC) targeting CD79b that delivers the cytotoxic microtubule inhibitor monomethyl auristatin E (MMAE); obinutuzumab, a glycoengineered Type II anti-CD20 monoclonal antibody; and a reduced-dose chemotherapy backbone known as miniCHP, which includes cyclophosphamide, doxorubicin, and prednisone. By omitting vincristine (the 'O' in standard CHOP), the regimen aims to reduce cumulative neurotoxicity while providing potent B-cell depletion through dual targeting of CD79b and CD20. This combination is often evaluated in clinical trials alongside novel targeted therapies, such as BTK inhibitors like orelabrutinib, to improve outcomes in newly diagnosed high-risk lymphoma patients.
