Polatuzumab vedotin + rituximab + cyclophosphamide + doxorubicin + orelabrutinib (Pola-RCHP-orelabrutinib) is an investigational chemoimmunotherapy combination regimen being evaluated for the treatment of newly diagnosed diffuse large B-cell lymphoma (DLBCL), particularly the MCD genetic subtype (characterized by MYD88 and CD79B mutations). The regimen combines the Pola-R-CHP backbone—consisting of the CD79b-targeted antibody-drug conjugate polatuzumab vedotin (Polivy), the anti-CD20 monoclonal antibody rituximab, and the cytotoxic agents cyclophosphamide and doxorubicin—with orelabrutinib, a potent, highly selective, covalent Bruton's tyrosine kinase (BTK) inhibitor developed by InnoCare Pharma. This combination aims to provide enhanced efficacy in subtypes of DLBCL that are highly dependent on B-cell receptor (BCR) and MYD88 signaling pathways. It is currently being studied in Phase 2 clinical trials led by investigators such as Ou Bai.
