Pola-ZRCHP is an investigational chemoimmunotherapy combination regimen being evaluated for the treatment of previously untreated diffuse large B-cell lymphoma (DLBCL), specifically targeting high-risk molecular subtypes such as MCD, BN2, and N1. The regimen builds upon the Pola-R-CHP backbone (polatuzumab vedotin, rituximab, cyclophosphamide, doxorubicin, and prednisone) by adding zanubrutinib, a potent and selective Bruton's tyrosine kinase (BTK) inhibitor. Polatuzumab vedotin is an antibody-drug conjugate (ADC) targeting CD79b, delivering the microtubule inhibitor monomethyl auristatin E (MMAE). Rituximab is an anti-CD20 monoclonal antibody. Cyclophosphamide and doxorubicin are standard cytotoxic chemotherapy agents. The addition of zanubrutinib aims to enhance efficacy by inhibiting B-cell receptor signaling, which is often constitutively active in specific DLBCL subtypes.
