Overview

Polatuzumab vedotin is an antibody-drug conjugate (ADC) targeting CD79b, a component of the B-cell receptor. It is composed of a humanized IgG1 monoclonal antibody conjugated to the microtubule-disrupting agent monomethyl auristatin E (MMAE). In the context of the Phase 2 study led by Weill Medical College of Cornell University, polatuzumab vedotin is combined with a modified R-EPOCH chemoimmunotherapy regimen—consisting of rituximab, etoposide, prednisone, cyclophosphamide, and doxorubicin (hydroxydaunomycin)—for the treatment of Richter's Transformation. Richter's Transformation is an aggressive progression of chronic lymphocytic leukemia (CLL) into diffuse large B-cell lymphoma (DLBCL). This combination aims to improve outcomes in this high-risk population by adding a targeted ADC to a backbone of intensive infusional chemotherapy.
