R-CEOP-90 is a combination chemotherapy regimen used primarily for the treatment of aggressive B-cell lymphomas, such as diffuse large B-cell lymphoma (DLBCL) and follicular lymphoma grade 3B. It is an anthracycline-free alternative to the standard R-CHOP regimen, designed for patients who have contraindications to anthracyclines like doxorubicin due to cardiac risk or other factors. The "90" in the name typically refers to a higher dose of etoposide compared to other CEOP regimens. The regimen consists of five drugs: - Rituximab (a monoclonal antibody targeting CD20 on B-cells) - Cyclophosphamide (an alkylating agent) - Etoposide (a topoisomerase II inhibitor) - Vincristine (a microtubule inhibitor) - Prednisone (a corticosteroid) Rituximab induces cell death in CD20-positive B-cells via immune-mediated mechanisms and direct cytotoxicity. Cyclophosphamide and etoposide disrupt DNA synthesis and repair, leading to apoptosis in rapidly dividing cells. Vincristine inhibits mitosis by binding tubulin, while prednisone exerts anti-inflammatory and lympholytic effects. This regimen is administered in cycles—typically every 21 days—with intravenous administration of rituximab, cyclophosphamide, vincristine, and etoposide on day 1; additional doses of etoposide may be given on subsequent days; prednisone is taken orally for several days per cycle[3][8]. Clinical studies suggest that R-CEOP regimens offer similar efficacy to R-CHOP for patients unable to tolerate anthracyclines[2][5].
