R-CHOP-14 is a dose-dense chemoimmunotherapy regimen used primarily for the treatment of diffuse large B-cell lymphoma (DLBCL). It consists of the monoclonal antibody rituximab combined with the CHOP chemotherapy backbone: cyclophosphamide, doxorubicin (hydroxydaunorubicin), vincristine (Oncovin), and prednisone. The '14' designation indicates that the treatment cycles are administered every 14 days with growth factor support, a schedule intended to improve efficacy compared to the standard 21-day cycle (R-CHOP-21). Rituximab targets the CD20 antigen on B-cells to induce cell lysis, while the chemotherapy components work synergistically to inhibit DNA synthesis, induce DNA damage, and disrupt microtubule formation. This specific regimen was evaluated by Nantes University Hospital in a randomized trial comparing it against high-dose chemotherapy with autologous stem cell support in younger adult patients with high-risk DLBCL.
