Overview

R-EPOCH + ibrutinib is a combination regimen used primarily in the treatment of relapsed or refractory diffuse large B-cell lymphoma (DLBCL), including AIDS-related lymphomas. The R-EPOCH regimen consists of rituximab (a monoclonal antibody targeting CD20), etoposide (a topoisomerase II inhibitor), prednisone (a corticosteroid), vincristine (a microtubule inhibitor), cyclophosphamide (an alkylating agent), and doxorubicin (an anthracycline antibiotic). Ibrutinib is a small molecule Bruton’s tyrosine kinase inhibitor that blocks B-cell receptor signaling pathways critical for the survival and proliferation of malignant B cells. The addition of ibrutinib to R-EPOCH aims to enhance antitumor activity by combining cytotoxic chemotherapy with targeted inhibition of B-cell signaling[1][3][5][6].
