R-GCVP

R-GCVP is a combination chemotherapy regimen consisting of rituximab, gemcitabine, cyclophosphamide, vincristine, and prednisolone. It was developed by University College, London and the UK National Cancer Research Institute as an alternative treatment for patients with newly diagnosed diffuse large B-cell lymphoma (DLBCL) who are considered unsuitable for the standard R-CHOP regimen, typically due to pre-existing cardiac dysfunction or risk factors that preclude the use of anthracyclines like doxorubicin. In this regimen, gemcitabine (a nucleoside analogue) replaces doxorubicin to reduce cardiotoxicity while maintaining anti-tumor activity. Rituximab provides targeted immunotherapy against CD20-positive B-cells, while cyclophosphamide, vincristine, and gemcitabine act as cytotoxic agents, and prednisolone serves as a lympholytic corticosteroid. A multicenter Phase II trial has evaluated the efficacy and safety of R-GCVP in this specific patient population.
