R-HDS (Rituximab-supplemented High-Dose Sequential chemotherapy) is an intensive chemo-immunotherapy regimen developed by the Gruppo Italiano Terapie Innovative nei Linfomi (GITIL) for the treatment of high-risk, previously untreated diffuse large B-cell lymphoma (DLBCL). The protocol integrates the anti-CD20 monoclonal antibody rituximab with a sequential high-dose chemotherapy schedule including cyclophosphamide, cytarabine (Ara-C), etoposide, cisplatin, mitoxantrone, and melphalan. This intensive approach is followed by autologous peripheral blood progenitor cell (PBPC) transplantation to rescue hematopoiesis. The regimen aims to overcome poor prognostic factors in patients with high age-adjusted International Prognostic Index (aaIPI) scores by maximizing dose intensity and utilizing the synergistic effects of targeted immunotherapy and multi-agent cytotoxic chemotherapy.
