Overview
R-MEGACHOP is a high-dose, dose-dense variant of the standard R-CHOP chemotherapy regimen used in the treatment of aggressive B-cell non-Hodgkin lymphoma, particularly diffuse large B-cell lymphoma (DLBCL). Like standard R-CHOP, it combines rituximab (a monoclonal antibody targeting CD20 on B cells), cyclophosphamide (an alkylating agent), doxorubicin (an anthracycline antibiotic), vincristine (a vinca alkaloid), and prednisone or prednisolone (a corticosteroid). The "Mega" designation refers to increased doses and/or shortened intervals between cycles compared to conventional regimens. The goal is to intensify therapy for high-risk patients. However, clinical studies have shown that increasing the dose intensity with regimens like R-MegaCHOP does not significantly improve outcomes compared to standard dose-dense approaches but does increase toxicity[6].
