Overview

Recombinant human interleukin 21 + rituximab is a combination therapy investigated for the treatment of indolent B-cell malignancies such as follicular lymphoma (FL), small lymphocytic lymphoma/chronic lymphocytic leukemia (SLL/CLL), and marginal zone lymphoma. Recombinant human interleukin-21 (rIL-21) is a cytokine that enhances immune effector function, particularly by activating natural killer (NK) cells and cytotoxic T cells. Rituximab is a monoclonal antibody targeting CD20 on B cells, leading to their depletion via mechanisms including antibody-dependent cellular cytotoxicity (ADCC). The combination aims to improve anti-tumor efficacy by leveraging rIL-21’s ability to enhance NK cell-mediated ADCC and direct antiproliferative effects on some B-cell malignancies. Clinical studies have shown that this regimen can be administered safely in an outpatient setting with evidence of durable remissions in some patients[1][2][3][4][6].
