**RELTpep-KLA exosomes** are an experimental, mesenchymal stem cell-derived exosome therapy engineered for targeted cancer treatment. The extracellular vesicles are loaded with KRAS-targeting small interfering RNA and surface-modified with RELTpep-KLA, a hybrid peptide comprising the RELT-binding sequence CRQTKN and the proapoptotic mitochondrial membrane-disrupting peptide KLAKLAKKLAKLAK. The construct is designed to bind receptor expressed in lymphoid tissues on tumor cells, deliver KRAS siRNA, reduce ERK phosphorylation, and induce tumor-cell cytotoxicity. In reported preclinical studies, intravenously administered RELTpep-KLA-targeted, KRAS siRNA-loaded MSC exosomes inhibited growth of Panc-1 pancreatic tumor xenografts and showed activity in A549 lung tumor and Panc-1 pancreatic tumor cells.
