Overview
This combination regimen, commonly referred to as R-CHOP plus methotrexate (R-CHOP-MTX), is an intensified immunochemotherapy protocol used for the treatment of diffuse large B-cell lymphoma (DLBCL), particularly in cases with central nervous system (CNS) involvement. The regimen consists of rituximab (an anti-CD20 monoclonal antibody), cyclophosphamide (an alkylating agent), doxorubicin (an anthracycline topoisomerase II inhibitor), vincristine (a microtubule inhibitor), prednisone (a corticosteroid), and methotrexate (an antimetabolite). In clinical trials conducted at Ruijin Hospital, this combination serves as a backbone therapy, often evaluated alongside novel agents such as the BTK inhibitor orelabrutinib to improve efficacy in high-risk DLBCL populations. The components work synergistically to induce apoptosis in malignant B-cells through immune-mediated mechanisms, DNA damage, and inhibition of cellular proliferation.
