Overview

This refers to an investigational combination treatment protocol developed by St. Petersburg State Pavlov Medical University for the treatment of anti-PLA2R antibody-positive primary membranous nephropathy. The regimen, referred to as RCP, consists of a single dose of rituximab (a CD20-targeting monoclonal antibody), four low-dose intravenous infusions of cyclophosphamide (an alkylating agent), and a tapering course of corticosteroids (methylprednisolone and prednisolone). The protocol aims to achieve early clinical remission while minimizing cumulative drug toxicity compared to standard-of-care regimens like the modified Ponticelli protocol. It works by depleting B-cells and suppressing the production of pathogenic anti-phospholipase A2 receptor (anti-PLA2R) antibodies that cause podocyte injury.
