This is an investigational multi-agent chemotherapy and immunotherapy regimen combining five drugs: rituximab (a monoclonal antibody targeting CD20), gemcitabine (a nucleoside analog antimetabolite), oxaliplatin (a platinum-based chemotherapeutic), bendamustine (an alkylating agent with purine analog properties), and polatuzumab vedotin (an antibody-drug conjugate targeting CD79b). The combination is being studied primarily for relapsed or refractory diffuse large B-cell lymphoma (DLBCL) in patients who are not eligible for stem cell transplant. Rituximab depletes B-cells by binding to CD20, while polatuzumab vedotin delivers a cytotoxic agent to cells expressing CD79b, leading to apoptosis. Gemcitabine inhibits DNA synthesis, oxaliplatin causes DNA crosslinking and damage, and bendamustine induces cell death through alkylation of DNA. This combination aims to improve outcomes in heavily pretreated DLBCL patients by leveraging multiple mechanisms of action[2][8].
