Overview

R-mabHDI is a combination chemotherapy regimen primarily utilized as a salvage therapy for patients with relapsed or refractory B-cell non-Hodgkin lymphoma (NHL). The regimen consists of the monoclonal antibody rituximab, which targets the CD20 antigen on the surface of B-cells, and high-dose ifosfamide (HDI), an alkylating agent that interferes with DNA replication. Rituximab works by inducing cell lysis through antibody-dependent cellular cytotoxicity (ADCC), complement-dependent cytotoxicity (CDC), and the induction of apoptosis. Ifosfamide, when administered at high doses, provides potent antineoplastic activity by forming DNA cross-links, which lead to cell cycle arrest and cell death. This combination is often employed to achieve remission in patients who have failed first-line treatments, such as CHOP or R-CHOP, and is sometimes used as a bridge to autologous stem cell transplantation.
