rituximab + methotrexate + MATRix

Rituximab + methotrexate + MATRix is a de-escalated induction chemotherapy regimen being evaluated in the Phase III OptiMATe trial for the treatment of newly diagnosed primary central nervous system lymphoma (PCNSL). This regimen consists of an initial pre-phase of rituximab (a CD20-targeting monoclonal antibody) and high-dose methotrexate (an antifolate), followed by two cycles of the full MATRix protocol, which includes rituximab, methotrexate, cytarabine, and thiotepa. The goal of this de-escalated approach is to reduce the high rate of infectious complications associated with the standard four-cycle MATRix regimen while maintaining therapeutic efficacy. The treatment is typically followed by consolidation with high-dose chemotherapy and autologous stem cell transplantation.
