Overview

This drug is a combination chemotherapy regimen consisting of five agents: rituximab (a monoclonal antibody targeting CD20), temsirolimus (an mTOR inhibitor), dexamethasone (a synthetic glucocorticoid corticosteroid), cytarabine (an antimetabolite chemotherapeutic agent), and cisplatin (a platinum-based DNA crosslinking agent). This specific combination, known as the STORM regimen, has been studied primarily in relapsed or refractory diffuse large B-cell lymphoma. The mechanism of action involves multiple pathways: - Rituximab targets CD20 on B-cells, leading to cell lysis via immune-mediated mechanisms. - Temsirolimus inhibits the mammalian target of rapamycin (mTOR) pathway, suppressing tumor cell growth and proliferation. - Dexamethasone exerts anti-inflammatory and lympholytic effects through glucocorticoid receptor agonism. - Cytarabine interferes with DNA synthesis by acting as a pyrimidine analog. - Cisplatin forms DNA crosslinks that inhibit replication and transcription. The STORM regimen was evaluated in a phase II clinical trial for patients with relapsed or refractory diffuse large B-cell lymphoma. The addition of temsirolimus to the standard R-DHAP protocol showed promising activity with manageable toxicity[1][2][4][5].
