rituximab + thiotepa + busulfan + cyclophosphamide (often abbreviated as R-TBC) is an intensive chemoimmunotherapy conditioning regimen developed and evaluated by Massachusetts General Hospital for the treatment of central nervous system (CNS) lymphomas. The regimen consists of the anti-CD20 monoclonal antibody rituximab combined with high-dose thiotepa, busulfan, and cyclophosphamide, followed by autologous stem cell transplantation (ASCT). This combination is specifically designed to address the challenges of treating CNS malignancies; while rituximab targets CD20-expressing B-cells, the alkylating agents thiotepa and busulfan are known for their excellent penetration of the blood-brain barrier. The regimen is used for both primary CNS lymphoma and secondary CNS involvement in non-Hodgkin lymphoma, aiming to achieve deep remission through myeloablative chemotherapy before hematopoietic rescue.
