Ropeginterferon alfa-2b + ruxolitinib is a combination therapy being investigated for the treatment of myelofibrosis, particularly in patients who have demonstrated a suboptimal response to ruxolitinib monotherapy. Ropeginterferon alfa-2b (Besremi) is a next-generation, long-acting monopegylated interferon-alpha-2b that acts as an agonist for the interferon-alpha/beta receptor (IFNAR), inducing antiproliferative and immunomodulatory effects on hematopoietic stem cells. Ruxolitinib (Jakafi/Jakavi) is a small-molecule inhibitor of Janus kinases 1 and 2 (JAK1/JAK2), which are often constitutively active in myeloproliferative neoplasms. The University of Hong Kong is sponsoring a Phase 2 clinical trial to evaluate the safety and efficacy of adding ropeginterferon alfa-2b to a stable regimen of ruxolitinib, aiming to achieve deeper molecular responses and improved clinical outcomes by simultaneously targeting the JAK-STAT pathway and the interferon signaling pathway.
