This combination prophylactic regimen is being investigated by Zhejiang University for the prevention of graft-versus-host disease (GVHD) in patients undergoing HLA-haploidentical hematopoietic stem cell transplantation (haplo-HSCT). The regimen consists of ruxolitinib, a Janus kinase (JAK) 1 and 2 inhibitor, administered in combination with a calcineurin inhibitor (such as tacrolimus or cyclosporine) and a short course of methotrexate. Ruxolitinib works by blocking the signaling of proinflammatory cytokines involved in the pathogenesis of GVHD. This triple-drug combination is being evaluated as an alternative to the standard mycophenolate mofetil (MMF)-based prophylaxis in the context of low-dose antithymocyte globulin (ATG) systems to reduce the incidence of acute and chronic GVHD.
