This combination therapy, often referred to as the RUXOPeg regimen, consists of the JAK1/JAK2 inhibitor ruxolitinib and the pegylated cytokine peginterferon alfa-2a. It was evaluated in a Phase 1/2 clinical trial (NCT02742324) sponsored by the French Innovative Leukemia Organisation (FILO) for the treatment of patients with primary or secondary myelofibrosis, including post-polycythemia vera and post-essential thrombocythemia myelofibrosis. Ruxolitinib works by inhibiting the Janus kinase signaling pathway, which is often overactive in myeloproliferative neoplasms, while peginterferon alfa-2a exerts antiproliferative and immunomodulatory effects. The combination aims to improve clinical outcomes and potentially achieve molecular responses by targeting different aspects of the disease biology through synergistic mechanisms.
