Overview

SAR3419 + rituximab is a combination therapy used for relapsed or refractory diffuse large B-cell lymphoma (DLBCL) and other B-cell malignancies. SAR3419 (coltuximab ravtansine) is an antibody-drug conjugate composed of an anti-CD19 humanized monoclonal antibody linked to the cytotoxic agent DM4 (a maytansinoid). The antibody selectively targets CD19, a surface molecule highly expressed on B-cell malignancies, and upon internalization releases DM4 to disrupt microtubule function, leading to cell cycle arrest and apoptosis. Rituximab is a monoclonal antibody targeting CD20, inducing cell death through multiple mechanisms, including antibody-dependent cellular cytotoxicity and complement-mediated lysis. This combination harnesses dual targeting of CD19 and CD20 on B-cells. Clinical trials showed moderate response rates and acceptable toxicity, but the program was terminated due to limited efficacy in heavily pretreated patients[1][3][4].
