This combination therapy consists of the selective inhibitor of nuclear export (SINE) selinexor, the Janus kinase (JAK) 1/2 inhibitor ruxolitinib, and the glucocorticoid methylprednisolone. Selinexor functions by binding to and inhibiting the nuclear export protein Exportin 1 (XPO1), which leads to the nuclear accumulation and activation of tumor suppressor proteins and the inhibition of oncoprotein mRNAs. Ruxolitinib is a potent inhibitor of JAK1 and JAK2, which are critical components of signaling pathways that promote cell proliferation and survival in hematologic malignancies, including multiple myeloma. Methylprednisolone provides anti-inflammatory and immunosuppressive effects and has demonstrated synergistic activity when combined with other myeloma therapies. This triplet regimen is being evaluated by Oncotherapeutics in a Phase I clinical trial for patients with relapsed or refractory multiple myeloma who have received at least three prior lines of therapy.
