This is a combination regimen consisting of six agents: - Selinexor, an oral selective inhibitor of nuclear export (SINE) that blocks exportin 1 (XPO1), leading to the accumulation and activation of tumor suppressor proteins and reduction in oncoproteins. - Rituximab, a monoclonal antibody targeting CD20 on B lymphocytes, mediating cell lysis via immune mechanisms. - Cyclophosphamide, an alkylating agent that crosslinks DNA to inhibit cell replication. - Doxorubicin, an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II. - Vincristine, a vinca alkaloid that disrupts microtubule formation during mitosis. - Prednisone, a synthetic glucocorticoid with anti-inflammatory and immunosuppressive effects. This combination is being investigated as frontline therapy for untreated diffuse large B-cell lymphoma (DLBCL) and other non-Hodgkin lymphomas. The addition of selinexor to the standard R-CHOP regimen aims to enhance antitumor activity by targeting XPO1-mediated resistance pathways. Early-phase clinical trials have shown encouraging efficacy with manageable toxicity profiles[1][2][6][9].
