T785-ISAC is an immune-stimulating antibody conjugate (ISAC) platform developed by Bolt Biotherapeutics and Stanford University, consisting of a proprietary Toll-like receptor 7/8 (TLR7/8) dual agonist payload, T785, conjugated to a tumor-targeting monoclonal antibody. The therapeutic is designed for systemic administration to convert "cold" tumors into "hot" tumors by activating the innate immune system within the tumor microenvironment. Its mechanism of action requires three key components: tumor antigen recognition by the antibody (e.g., HER2 or CD20), Fcγ-receptor-mediated phagocytosis by myeloid cells such as macrophages and dendritic cells, and subsequent TLR7/8-mediated activation of these cells. This process triggers the release of pro-inflammatory cytokines and chemokines, promotes antigen cross-presentation, and recruits T cells to establish a durable, systemic antitumor immune response and immunological memory.
