Overview

Tacrolimus + vedolizumab is a combination therapy used primarily in the management of moderate to severe inflammatory bowel diseases (IBD), including ulcerative colitis and Crohn's disease. Tacrolimus is a calcineurin inhibitor that suppresses T-cell activation by inhibiting the phosphatase activity of calcineurin, thereby reducing cytokine production and immune response. Vedolizumab is a humanized monoclonal antibody targeting the α4β7 integrin on lymphocytes; it blocks their interaction with mucosal addressin cell adhesion molecule-1 (MAdCAM-1) on gut endothelial cells, selectively inhibiting lymphocyte trafficking to gastrointestinal tissue and reducing gut inflammation. The combination has been shown in clinical studies to be more effective than either agent alone for inducing remission and reducing immune cell infiltration in patients with refractory IBD[1][2][5]. Tacrolimus is typically used as an induction agent for rapid disease control while vedolizumab serves as maintenance therapy due to its favorable safety profile[5][7].
