Overview

Tocilizumab + ruxolitinib is a combination therapy consisting of two distinct agents used primarily for the management of severe inflammatory and immune-mediated conditions, particularly in cases refractory to standard treatments. Tocilizumab is a recombinant humanized monoclonal antibody targeting the interleukin-6 receptor (IL-6R), thereby inhibiting IL-6-mediated pro-inflammatory signaling. Ruxolitinib is an oral small molecule inhibitor of Janus kinase 1 and 2 (JAK1/2), blocking downstream cytokine signaling involved in immune cell activation and proliferation. This combination has been explored for steroid-refractory graft-versus-host disease (GVHD) after hematopoietic stem cell transplantation, as well as for severe cytokine release syndrome (CRS) following CAR-T therapy or other hyperinflammatory states where single-agent therapy was insufficient[1][5]. The rationale behind this combination lies in their complementary mechanisms—tocilizumab directly blocks IL-6-driven inflammation, while ruxolitinib broadly suppresses multiple cytokine pathways via JAK inhibition.
