Overview

Tofacitinib is an oral small molecule Janus kinase (JAK) inhibitor used primarily for the treatment of autoimmune and inflammatory conditions such as rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, polyarticular course juvenile idiopathic arthritis, and ulcerative colitis. It works by inhibiting JAK1 and JAK3 enzymes in the JAK-STAT signaling pathway, thereby reducing immune system activity and inflammation[1][5][7]. Tofacitinib was developed by Pfizer in collaboration with the National Institutes of Health. Rifampin (also known as rifampicin) is a broad-spectrum antibiotic mainly used for treating tuberculosis and other mycobacterial infections. It acts by binding to the beta subunit of bacterial DNA-dependent RNA polymerase (RNAP), inhibiting RNA synthesis in bacteria[6][8]. Rifampin is also known under brand names such as Rifadin and Rimactane. When combined (tofacitinib + rifampin), these drugs are not intended as a fixed-dose combination but may be co-administered in clinical scenarios. However, significant drug-drug interactions exist; notably, rifampin induces cytochrome P450 enzymes that can reduce plasma concentrations of tofacitinib.
