Topoxy is a topical corticosteroid formulation containing 0.25% desoximetasone, a high-potency (Class II) synthetic glucocorticoid. It is primarily indicated for the treatment of inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses, such as scalp psoriasis. Desoximetasone works by diffusing across cell membranes and binding to the cytoplasmic glucocorticoid receptor. This complex then translocates to the cell nucleus, where it binds to specific DNA sequences to modulate gene expression. This results in the induction of anti-inflammatory proteins (e.g., lipocortins) and the inhibition of pro-inflammatory mediators, including cytokines, prostaglandins, and leukotrienes, thereby reducing vasodilation, capillary permeability, and leukocyte migration to the site of inflammation.
