Venetoclax + tofacitinib is an investigational drug combination being evaluated for the treatment of relapsed or refractory T-cell acute lymphoblastic leukemia (T-ALL). Venetoclax is a small molecule inhibitor of B-cell lymphoma 2 (BCL-2), an anti-apoptotic protein often overexpressed in leukemic cells. Tofacitinib is a small molecule Janus kinase (JAK) inhibitor, specifically targeting JAK3 and JAK1, which inhibits the IL7R/JAK-STAT signaling pathway. This pathway is frequently dysregulated in T-ALL and contributes to cell survival and proliferation. The combination is being studied in precision medicine trials, such as the ALL-TARGET trial led by Philippe Rousselot, where patients are selected based on BCL2 expression and IL7R pathway status.
