**Venetoclax + obinutuzumab** is a fixed-duration, chemotherapy-free combination regimen approved for the first-line treatment of adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL)[1][4][5][7]. **Venetoclax** is an orally bioavailable, selective small molecule inhibitor of B-cell lymphoma 2 (BCL-2), an anti-apoptotic protein overexpressed in CLL and other malignancies[2][4]. By inhibiting BCL-2, venetoclax promotes apoptosis of malignant B-cells. **Obinutuzumab** is a glycoengineered type II monoclonal antibody targeting the CD20 antigen on B-lymphocytes, enhancing antibody-dependent cellular cytotoxicity (ADCC), direct cell death, and immune-mediated killing of B-cells[4][6]. The combination produces higher response rates and longer progression-free survival than standard chemoimmunotherapy, with a favorable safety profile and fixed treatment duration (12 cycles)[1][4][7].
