This is a combination therapy regimen that includes venetoclax, rituximab, and components of the CHOP regimen (cyclophosphamide, doxorubicin, vincristine, and prednisone). Venetoclax is a highly selective oral inhibitor of the BCL-2 anti-apoptotic pathway, which is constitutively overexpressed in CLL clones. It works by targeting BCL-2, preventing it from working, which normally prevents cancer cells from dying. Rituximab is a humanized murine monoclonal antibody targeting CD20 on mature B lymphocytes. The combination of venetoclax and rituximab has shown significant efficacy in treating chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL), with the MURANO trial demonstrating an 83% reduction in the risk of disease progression compared to bendamustine plus rituximab. The addition of CHOP components (cyclophosphamide, doxorubicin, vincristine, and prednisone) likely aims to enhance the therapeutic effect for certain lymphoid malignancies.
