Overview

Vorasidenib + temozolomide is an investigational combination therapy being developed by Servier for the treatment of IDH-mutant gliomas. Vorasidenib is a first-in-class, oral, brain-penetrant dual inhibitor of the mutant isocitrate dehydrogenase 1 and 2 (IDH1/IDH2) enzymes. By inhibiting these mutant enzymes, vorasidenib reduces the accumulation of the oncometabolite 2-hydroxyglutarate (2-HG), which is responsible for epigenetic dysregulation and blocked cellular differentiation in glioma cells. Temozolomide is an imidazotetrazine derivative and an alkylating chemotherapy agent that crosses the blood-brain barrier and exerts its cytotoxic effect by methylating DNA at the O6 and N7 positions of guanine, leading to DNA strand breaks and apoptosis. The combination is currently being evaluated in clinical trials to assess its safety and efficacy in patients with grade 2, 3, and 4 IDH-mutant astrocytomas or oligodendrogliomas, aiming to provide a synergistic therapeutic approach for higher-grade or recurrent disease.
