The combination of vorinostat, cladribine, and rituximab (known as the SCR regimen) is an investigational chemoimmunotherapy treatment for B-cell malignancies. Vorinostat is an oral small molecule histone deacetylase (HDAC) inhibitor that promotes histone acetylation, leading to altered gene expression, cell cycle arrest, and apoptosis. Cladribine is a purine nucleoside analog that acts as an antimetabolite, interfering with DNA synthesis and repair, particularly in lymphoid cells. Rituximab is a chimeric monoclonal antibody directed against the CD20 antigen found on the surface of normal and malignant B-lymphocytes, inducing cell death through antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). This triplet combination was specifically evaluated by the OHSU Knight Cancer Institute to exploit potential synergies between epigenetic modulation and standard chemoimmunotherapy in diseases like mantle cell lymphoma and chronic lymphocytic leukemia.
