Overview

Vorinostat + R-CHOP is an investigational combination regimen consisting of the histone deacetylase (HDAC) inhibitor vorinostat and the standard immunochemotherapy R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone). This regimen was primarily evaluated in the SWOG S0806 phase I/II clinical trial sponsored by the National Cancer Institute (NCI) for the treatment of patients with newly diagnosed, advanced-stage diffuse large B-cell lymphoma (DLBCL). Vorinostat acts as an epigenetic modulator that inhibits HDACs, potentially enhancing the expression of major histocompatibility Class II (MHCII) antigens and sensitizing cancer cells to the cytotoxic and pro-apoptotic effects of the R-CHOP components. The trial explored whether the addition of vorinostat could improve the efficacy of the standard-of-care R-CHOP backbone, though the vorinostat dosing schedule required adjustment during the study due to dose-limiting toxicities.
