Zandelisib + rituximab is a combination regimen under investigation for the treatment of relapsed or refractory B-cell malignancies, specifically indolent non-Hodgkin’s lymphomas such as follicular lymphoma (FL) and marginal zone lymphoma (MZL)[1][3]. Zandelisib is a selective phosphatidylinositol 3-kinase delta (PI3Kδ) inhibitor that blocks the PI3Kδ-mediated AKT signaling pathway, resulting in inhibition of tumor cell proliferation and survival in B-cell malignancies[4]. Rituximab is a monoclonal antibody targeting CD20 on B lymphocytes, leading to cell death via direct cytotoxicity, complement activation, and antibody-dependent cell-mediated cytotoxicity. The combination harnesses zandelisib’s targeted kinase inhibition and rituximab’s immunotherapeutic B-cell depletion, aiming for enhanced antitumor efficacy. Zandelisib is orally administered, typically at 60 mg once daily in an intermittent dosing schedule, while rituximab is administered intravenously during the first six months of combination therapy[2][5].
