Overview

The combination of zanubrutinib, obinutuzumab, and lenalidomide (ZGR) is a novel targeted therapy regimen being investigated for various lymphoid malignancies. This chemotherapy-free combination brings together three distinct mechanisms of action to potentially improve outcomes in patients with certain types of lymphoma and leukemia. The ZGR regimen combines zanubrutinib (a next-generation Bruton tyrosine kinase inhibitor), obinutuzumab (a third-generation CD20 monoclonal antibody), and lenalidomide (an immunomodulatory drug). This combination is designed to deepen remission in patients with lymphoid malignancies while avoiding cytotoxic chemotherapeutic agents, potentially improving treatment tolerance, especially in elderly patients or those with poor tolerance[5]. The safety profile of the ZGR regimen is still being established through ongoing clinical trials. However, data from studies of zanubrutinib plus obinutuzumab suggest that this combination is generally well-tolerated[6][7]. Common adverse events observed with zanubrutinib-based regimens include thrombocytopenia, neutropenia, diarrhea, fatigue, upper respiratory tract infections, and contusions[9]. The addition of lenalidomide to this combination is being evaluated for both efficacy and safety in current clinical trials.
