This is an investigational combination therapy being evaluated at Shanghai Ruijin Hospital for the treatment of newly diagnosed primary central nervous system lymphoma (PCNSL). The regimen consists of zanubrutinib, a second-generation Bruton tyrosine kinase (BTK) inhibitor; rituximab, an anti-CD20 monoclonal antibody; and high-dose methotrexate, a folate antagonist chemotherapy. In patients harboring TP53 mutations, the XPO1 inhibitor selinexor is added to the regimen. This biomarker-driven approach aims to improve outcomes in PCNSL, an aggressive B-cell lymphoma, by combining targeted therapy, immunotherapy, and traditional chemotherapy to enhance central nervous system penetration and overcome resistance mechanisms.
