Drug intelligence / Profile preview
The combination of zecdarasib (INCB161734) with folinic acid, fluorouracil, irinotecan, and oxaliplatin is an investigational antineoplastic regimen designed to treat KRAS G12D-mutated malignancies, particularly pancreatic ductal adenocarcinoma (PDAC). Zecdarasib is a novel, selective, oral small molecule ON/OFF inhibitor that targets both the active (GTP-bound) and inactive (GDP-bound) states of the mutant KRAS G12D protein, thereby blocking downstream oncogenic signaling pathways such as RAF-MEK-ERK and PI3K-AKT. The chemotherapy backbone, known as modified FOLFIRINOX (mFOLFIRINOX), consists of oxaliplatin (a DNA cross-linking agent), irinotecan (a topoisomerase I inhibitor), and fluorouracil (an antimetabolite whose thymidylate synthase inhibition is biochemically enhanced by folinic acid). Developed by Incyte, this combination regimen is being evaluated in Phase 1 and Phase 3 clinical trials to improve survival outcomes in patients with advanced or metastatic KRAS G12D-mutated solid tumors.
