Drug intelligence / Profile preview
**Zoldonrasib + folinic acid + fluorouracil + irinotecan + oxaliplatin** (also known as zoldonrasib in combination with modified FOLFIRINOX or mFOLFIRINOX) is an investigational combination regimen being developed by Revolution Medicines for the first-line treatment of patients with metastatic KRAS G12D-mutated pancreatic ductal adenocarcinoma (PDAC). Zoldonrasib (RMC-9805) is an oral, mutant-selective, covalent tri-complex inhibitor of the active, GTP-bound state of KRAS G12D (KRAS G12D(ON)). It acts as a molecular glue by first binding to the cellular chaperone cyclophilin A to form a binary complex, which then selectively binds and covalently inhibits KRAS G12D, blocking downstream oncogenic signaling. This targeted agent is combined with modified FOLFIRINOX—a standard-of-care chemotherapy regimen consisting of folinic acid (leucovorin), fluorouracil (5-FU), irinotecan, and oxaliplatin—to provide synergistic antitumor activity. The combination is currently being evaluated in the Phase 3 RASolute 305 trial for first-line metastatic KRAS G12D-mutated pancreatic cancer, following promising Phase 1/2 data demonstrating high objective response rates and deep molecular responses.
