Overview

Zorifertinib (AZD3759) is a potent, reversible, and central nervous system (CNS)-penetrant EGFR tyrosine kinase inhibitor (TKI) specifically designed to cross the blood-brain barrier. This combination therapy involves the oral administration of zorifertinib alongside chemotherapy (such as pemetrexed or methotrexate) delivered directly into the cerebrospinal fluid (CSF) via an Ommaya reservoir. It is primarily investigated for the treatment of leptomeningeal metastasis (LM) in patients with EGFR-mutant non-small cell lung cancer (NSCLC) who have progressed on prior EGFR-TKI therapies, including third-generation inhibitors like osimertinib. By combining a CNS-penetrant TKI with direct CSF-delivered chemotherapy, the regimen aims to maximize therapeutic concentrations in both the brain tissue and the fluid compartments of the central nervous system to overcome resistance and manage leptomeningeal disease.
