Overview

The cutaneous immune system (CIS) is a specialized, multi-layered network of cells and molecular signals located within the skin that serves as a critical interface between the body and the environment. It consists of diverse cell populations, including keratinocytes, Langerhans cells, dermal dendritic cells, and various T-cell subsets, which work in concert to provide immune surveillance and maintain tissue homeostasis (Nestle et al., 2009). The CIS plays a dual role: it protects against pathogens and physical damage while also being the primary site of dysregulation in chronic inflammatory conditions like psoriasis and atopic dermatitis (StatPearls, 2023). Because the CIS is a complex physiological system rather than a single protein or receptor, it is not considered a discrete molecular target; instead, it represents a therapeutic landscape where specific molecules, such as cytokines or receptors, are targeted to treat skin-related pathologies (Bos, 2004). Understanding the CIS is fundamental for developing biologics and small molecules that can precisely modulate skin immunity without compromising systemic health.
