Overview

The host skin microenvironment is a complex, multi-layered ecosystem comprising various cell types (keratinocytes, fibroblasts, immune cells), the extracellular matrix, and a diverse community of microorganisms known as the skin microbiota (Byrd et al., 2018). It serves as the body's primary physical and immunological barrier against environmental pathogens, UV radiation, and chemical insults while regulating water loss and temperature (StatPearls, 2023). This microenvironment is characterized by a specific acidic pH and a delicate balance of antimicrobial peptides and cytokines that coordinate local inflammatory responses (Guttman-Yassky et al., 2017). Dysregulation of this microenvironment, including shifts in microbial composition (dysbiosis) or aberrant immune signaling, is central to the pathogenesis of inflammatory skin diseases like atopic dermatitis and psoriasis, as well as skin malignancies (Nature Reviews Disease Primers, 2018). While not a single molecular target, therapeutic strategies often aim to modulate the microenvironment as a whole through topical agents that restore barrier function or systemic biologics that target specific inflammatory mediators within the cutaneous niche (Journal of Investigative Dermatology, 2020).
