Hyaluronidase PH-20, also known as Sperm adhesion molecule 1 (SPAM1), is a human enzyme that plays a critical role in the degradation of hyaluronan, a major polysaccharide of the extracellular matrix (UniProt P38567). Naturally, it is found on the sperm surface and in the lysosome, where it facilitates fertilization by allowing sperm to penetrate the hyaluronan-rich cumulus layer surrounding the oocyte (PubMed PMC2844457). In clinical practice, recombinant human hyaluronidase PH-20 (rHuPH20) is used as a spreading factor to increase the dispersion and absorption of other injected drugs (FDA Hylenex Label). By temporarily hydrolyzing hyaluronan at the injection site, it lowers the viscosity of the interstitial matrix, enabling the subcutaneous administration of large-volume biologics and fluids (Halozyme ENHANZE Technology). This technology is widely employed in oncology and immunology to transition treatments from intravenous to subcutaneous routes, improving patient convenience and healthcare efficiency. Additionally, PH-20 has been explored as a therapeutic agent in cancers characterized by high hyaluronan levels, which contribute to elevated interstitial fluid pressure and drug resistance (PubMed PMC5439107).
